    U.S. Service Member Disability Benefit Evaluations

    We provide disability benefit evaluations to both active duty and veteran service members. In order to schedule your evaluation, you may contact us via email, phone, or schedule a consultation through our website. Dr. Reynolds will conduct a brief consultation with you, during which she will explain what to expect from the process. At the end of the consultation, she will schedule you with one of our licensed psychologists. Here are a few things she will cover during your phone call:

    • The evaluation will consist of one 30 minute appointment which will be virtual. We do run on time so we ask that you are also on time. In the days leading up to it, you will receive a link to the appointment. We suggest that you click on that link when you receive it in order to make sure that it works from your device ahead of time.
    • Dr. Reynolds will ask you what state you reside in as we want to make sure we are licensed in your state prior to the evaluation. You can also verify that we are licensed in your state by navigating to our home page and scrolling down to the section “states we are licensed to practice psychology in.”
    • After your phone consultation with Dr. Reynolds, she will send you our new client paperwork. It is all electronic and mostly consent forms that ask for your signature. There is one form in there that is our service member questionnaire. It’s going to ask you about the symptoms that you are having and their impact on your daily life. Please put as much information as possible into that questionnaire as it will help inform your evaluation.
    • The fee for the evaluation is $350 and will be due at the time of service. If you need to change or cancel your appointment, we ask that you give us at least 24 hours notice; otherwise, there is a $150 late cancellation fee. Unfortunately we are out of network for all insurance companies; however, upon request, we would be happy to provide you with a superbill that you can then provide to your insurance company for possible reimbursement.
    • After the evaluation, your clinician will complete the disability benefits questionnaire (DBQ) for you. It takes us about a week to do that. At that point, we will send the DBQ over to your referring provider (if you don’t have one, we’d be happy to help you find one). We will also send you an email letting you know that we’ve done that.
